During sex, men have premature ejaculation when semen leaves their bodies earlier than expected. Sexual complaints such as premature ejaculation are common. The prevalence of the disease is as high as 1 in 3 people. When it doesn’t happen often, premature ejaculation isn’t a concern. Human sexuality, however, is primarily about pleasure, so men have learned how to control ejaculation to enhance their own and their partner’s enjoyment. Considering these considerations, it can be hypothesized that ejaculation control is a cultural rather than a natural phenomenon.

It is important to note that PE is not only a psychological disorder but also a neurobiological one. Considering PE holistically allows it to be classified as a psycho-neuro-endocrine and urologic condition. It is possible that the dopaminergic pathway stimulates ejaculation through the D2 receptors. In the male genital tract, the parasympathetic sacral outflow innervates the striated muscles through the pudendal nerve, causing clonic contractions. Hormones play an important role in ejaculatory function. Oxytocin and vasopressin, neurohypophysial hormones, are both involved in the regulation of male genital tract contractility. Men’s sexuality is governed by testosterone, which contributes to their gender identity, their sexual desire, and to their erections and ejaculations. PE has been inconsistently associated with low serum testosterone levels. In some patients with PE, treatment with dapoxetine significantly improves symptoms, but with modest efficacy. Due to the high heterogeneity of research data, it remains unclear whether SSRIs, TAs, and tramadol are effective.

Research on modern acupuncture has discovered that acupuncture stimulates the release of endogenous opioid endorphins. In addition to establishing the neural model of the acupuncture mechanism, this discovery is significant. In addition to generating nerve signals to the brain that result in the release of endorphins, acupuncture stimulation may also activate other neural pathways and release other neurotransmitters. The effects of acupuncture on neurotransmitters such as serotonin and dopamine have been shown in studies to influence emotional states and cravings. Multiple hormones related to sexual function may be released through the hypothalamus-pituitary and testicle axes after acupuncture. Acupuncture may treat PE by utilizing this mechanism. Based on the above-mentioned mechanism, acupuncture stimulation can simultaneously regulate multiple neurotransmitters and hormones. For PE, acupuncture may be a more effective treatment.
